Building Sustainable primary Health Care

As the global landscape continues to evolve, systems of primary health care must develop in a sustainable way. The Astana declaration emphasizes the need for comprehensive, core public health functions, delivered in a way that is integrated, not fragmented. On a systems level, recent years have seen greater emphasis on capacity building, strengthening supply chains, and bolstering mechanisms to ensure quality and safety. Additionally, efforts have been made to better integrate PHC and vertical programs. Taking place concurrently has been the emergence of novel health technologies. Digital technologies have become essential resources in building a primary health care that is sustainable and that provides services that are of high quality and safety. However, their advantages and disadvantages are often points of debate. During the conference, we hope to hear from a diverse array of experiences and perspectives on e-health, with the aim of better understanding how to efficiently and effectively utilize e-health in a way that truly strengthens the system. We will also feature country experiences of efforts to move in meaningful ways towards comprehensive, sustainable PHC.

Experience with using DHIS2 to empower health workers in Primary Health Care.

The recognized global public good, DHIS2 was developed as an open-source platform aimed at providing accessible, adaptable health information systems through local ownership, capacity-building and innovation. With the DHIS2 Individual Tracker, the platform is able to support community health workers gather quality individualised data for timely decision making both at the local and national level. Contributing to the transition to digitised health data, DHIS2 can play a role in building sustainable and equitable primary health care systems. -An overview of the development of the Individual Tracker and the HISP model to build local capacity for development and maintenance of digital health information systems.

A Twinning Partnership Program to Strengthen Capacity for Policy and Practice of Primary Health Care: Effectiveness and Sustainability

The University of Queensland (UQ) in Australia and the International Institute for Primary Health Care- Ethiopia (IPHC-E) have been implementing a twinning partnership program, with the support of the Bill and Melinda Gates Foundation through the American International Health Alliance (AIHA), since January 2022. The purpose of this twinning partnership program is to capacitate the Institute (and its hubs) so that it becomes a centre of excellence and a World Health Organization’s (WHO’s) collaborating centre in primary health care (PHC). We have learned several lessons for effective and sustainable twinning program to strengthen institutional capacity for PHC. The objective of this session is to provide an overview of the twinning partnership program, including its strategies, activities and achievements, and summarize the lessons learned towards an effective and sustainable technical assistance program.

Redesigning Referral experience from Primary Healthcare level in India : A Behavioral Science and Human Centered Design Approach

To achieve the goal of universal health coverage, the Government of India has rolled out a comprehensive primary healthcare strategy in the country. In alignment with GoI priorities, PATH, has adopted a district saturation approach for demonstration of PHC in India. To understand behavioral barriers faced by the demand and supply side, a behavior science and human-centered design (HCD) approach was implemented across all elements of health systems for reimagining primary healthcare delivery.

Supporting home grown technology solutions for primary health care in Sierra Leone

There is an increasing gap in the global footprint of technological advances and innovations which remains dominated by global north countries.In 2022 Concern Sierra Leone partnered with Autohealth (SL) Limited, a tech startup founded by medical students with a drive to solve key issues in the health system. They developed a Safe Blood Management Information System (SBMIS) – LifeBlood - as well as a ‘Community Mobile App’. The LifeBlood platform digitizes current paper forms and working documents of the National Safe Blood Service and automates the various work streams and creates donor to transfusion tracking. Whereas the app aims to increase the number of voluntary blood donations in Sierra Leone. This collaboration is an example of local solutions for primary health service delivery gaps and evidence to support increased investment in local start up technology companies to provide sustainable solutions to key primary health and development obstacles.

An NCD medicine and product forecasting tool providing evidence for stronger supply chains

Gaps in the ability to forecast for the demand of medicines cause challenges for everyone from the government decision makers to patients. In response to this need, the Coalition for Access to NCD Medicines & Products developed an NCD Forecasting Tool for Essential Medicines and Products and accompanying toolkit, in partnership with the ministries of health in Kenya and Uganda. During this session, multisectoral members from the Coalition will explain why they developed the NCD forecasting program, lessons learned from its pilot, and its impact, both in country and globally.

Ethiopian Capitation Pilot Experience: setting an appropriate capitation rate for Ethiopian context

To address challenges related to the current fee-for-service provider payment modality, the EHIS with support from CHAI piloted capitation over two years. This study outlines the considerations made and lessons learned in designing a fit-for-context capitation model. The aim is to compare the performance of the two alternative designs of capitation based on accuracy, and suitability for scale-up.

Building back better: Supporting delivery of early childhood development and maternal mental health services as part of rebuilding primary health care after conflict in northern Amhara, Ethiopia

Background: Primary health care (PHC) has been affected by conflict in northern Amhara Region, Ethiopia. Conflict also affects early childhood development (ECD) and maternal mental health (MMH). As part of PHC rebuilding, facility-level providers were trained at 18 health centers and 81 health posts in six woredas. A multifaceted approach to integrate ECD and MMH into post-conflict PHC rebuilding is feasible and can improve key mental health and development outcomes.

The strength of primary health care is rooted in an empowered people and workforce. The rights and protections of primary health care (PHC) workers play a crucial role in guaranteeing quality healthcare services. Equity and gender concerns are important to consider for the PHC workforce and there is a need for more involvement from young people in the sector. As Community Health Workers remain the backbone of the delivery of PHC in many settings, how do we ensure effective methods of recruitment, training, and retention that meet the needs of the workforce? Burkina Faso has experienced a humanitarian crisis born of armed conflicts since 2015, coupled with the 2019 Covid-19 outbreak straining the functioning of the health system. Community Engagement: an important piece in improving PHC service delivery

“Three Delays" model proposes that pregnancy-related mortality is overwhelmingly due to delays in: (1) deciding to seek appropriate medical help for an obstetric emergency; (2) reaching an appropriate obstetric facility; and (3) receiving adequate care when a facility is reached. To address the three delays, Pathfinder international, is implementing the Saving Mothers Giving Life 2.0 (SMGL 2.0) project in Kaduna state, Nigeria with the goal of contributing to the state government’s efforts to reduce maternal and perinatal mortality within the state.

Align stakeholders support with national policies, strategies and plans

PHC financing faces several challenges, including a low level of public funding, fragmented financing mechanisms, limited coverage for patients, and a lack of financial incentives for PHC providers to deliver priority services. The establishment of a robust primary health care financing system is dependent upon the multi-sectoral collaboration of various stakeholders including donors, political actors, and the community. It is important also to consider the role and responsibilities that each party may have. For example, to what extent do donors collaborate to support national leadership? How might widely different stakeholders interact and contribute meaningfully to improve primary health care? ICPHC aims to explore ways these stakeholders’ support can be effectively aligned with national policies, strategies and plans in order to improve health outcomes. We hope country representatives use this platform to share their experience and lessons that can be adapted and used by other countries to guide implementation of effective PHC financing.
